Authentication failure during token refresh for Azure SQL Database Managed Identities

Implementing least-privilege access in Azure SQL Database often involves utilizing Microsoft Entra ID Managed Identities to remove hardcoded credentials. In this architecture, the application relies on OAuth2 tokens that possess a finite lifespan and must be refreshed to maintain a persistent connection to the database engine.
A challenge arises when the identity's credentials expire or the token refresh mechanism fails at the gateway level. Because the authentication occurs before the request reaches the SQL engine, it is unclear how the database handles existing sessions when the underlying identity token expires during a long-running transaction or a persistent connection pool.

The Azure SQL Database gateway does not terminate active sessions immediately when a managed identity token expires. Existing connections remain usable until the next operation that requires re‑authentication. When the token cannot be refreshed, the gateway returns an authentication error on the next login attempt, causing the connection pool to discard the faulty connection and attempt to create a new one.
Long‑running transactions or idle connections that outlive the token are not interrupted by the gateway. However, any subsequent batch, commit, or new query after the token has expired will trigger re‑authentication and fail with an authentication error, potentially causing the transaction to roll back. Connection‑pool libraries (e.g., ADO.NET SqlConnectionPool) treat the failed connection as invalid, remove it from the pool, and try to open a new connection, which forces a fresh token acquisition. If token refresh continues to fail, the pool may exhaust and surface authentication exceptions to the application.
WAITFOR DELAY '00:10:00').If your tenant has Azure AD Conditional Access policies that enforce token re‑validation at the gateway, the gateway may terminate sessions immediately on token expiry. Confirm whether such Conditional Access policies are enabled for the managed identity; this detail changes the recommendation (you would need to adjust token lifetime or policy settings).
